Important Info About UHC Reward Credit Card
If you or your dependents are enrolled in one of the AT&T Advantage Plans you should receive a debit card for reward dollars (real money) to be applied to that specific card. You will need to activate the card and participate in the Reward Program Activities.
There is no mandatory requirement to do so.
Should you or your dependent activate the card and do one or all of these eligible activities the VISA card sent to you will be credited with the associated amount each time.
